As a Federally Qualified Health Center, Aunt Martha’s Health and Wellness provides services to the most vulnerable populations regardless of their ability to pay. We are committed to supporting the well-being of our communities, ensuring equity in access and delivering exceptional care driven by a culture of innovation. Our services are available to individuals with health insurance and those without health insurance for a small fee, based on their income. Aunt Martha’s also assists in enrolling patients in health coverage for which they are eligible. Our 12 health centers address major healthcare needs–asthma, diabetes, hypertension, obesity, prenatal care, and behavioral health. Through our established processes for screening and referrals to address Social Drivers of Health (SDoH), Aunt Martha’s seeks to address multifaceted needs within the communities we serve. Our Integrated Care model enables us to assess our patients’ complex and varied health and social needs and meet them holistically. We are committed to identifying and addressing all of our patients’ needs via cross-integration, where patients enter our care network via one service and gain access to many additional services through our Integrated Care model. Coordinating care ensures that individuals will receive comprehensive services that address their medical, social, and behavioral health needs, as well as social drivers that impact their health in a multitude of ways.
